CLL Umpire Incident Report
CLL requires this fillout within 24 hours of any notable incidents, including games with rule violations, and other events worthy of commentary, such as the disqualification/ejections of any trainer, manager, coach, or player.
INJURY REPORTING
This form is for Local League Use ONLY.
The purpose of this form is to track incidents and be used to evaluate potential safety hazards, unsafe practices and/or to contribute positive ideas in order to improve league safety. When an accident occurs, please obtain as much information as possible.
This report is submitted to the CLL President and the CLL Safety Coordinator.

This form is for Local League Use ONLY.
The purpose of this form is to track incidents and be used to evaluate potential safety hazards, unsafe practices and/or to contribute positive ideas in order to improve league safety. When an accident occurs, please obtain as much information as possible.
The concussion Protocol form will also be used to track player eligibility in conjunction with the Safety Coordinator, The Parents or Guardians of the underaged player and the players medical treatment and professional assessment(s).
This report is submitted to the CLL President and the CLL Safety Coordinator.
